1. Hemorrhoids / Piles (Arshas)

Hemorrhoids involve the pathological dilatation and inflammation of the submucosal veins situated within the anal canal. They are divided into internal and external presentations, requiring careful staging:

The Four Degrees of Internal Hemorrhoids:

• First Degree (Stage 1): No physical tissue mass prolapses outside the anal canal. The primary clinical indicators are itching and active rectal bleeding that appears as a distinct splash on the toilet pan.
• Second Degree (Stage 2): The internal pile mass actively prolapses out of the anus during defecation but returns inside spontaneously on its own immediately afterward.
• Third Degree (Stage 3): The pile mass emerges prominently during bowel movements but lacks the elasticity to return spontaneously; it remains outside until it is manually pushed back inside by the patient.
• Fourth Degree (Stage 4): The structural pile mass remains permanently prolapsed outside the anal sphincter and cannot be reduced, even with manual attempts. Chronic bleeding across these stages can lead to secondary Anemia from sustained blood loss.

External Hemorrhoids: These develop directly on the outer anal margin and are clinically characterized by sudden, localized swelling, intense pain, and acute tenderness.

Ayurvedic Etiology (Causes) of Arshas: Triggered directly by compromised digestive fire (Mandagni), heavy manual labor, continuous sitting on hard or irregular surfaces, frequent voluntary straining (bearing down), and the forceful suppression or induction of natural bodily urges (such as defecation, flatulence, and urination). It is also highly prevalent following sudden weight loss and during or after pregnancy.

2. Anal Fissure (Parikarthika)

An anal fissure is a sharp mechanical tear or longitudinal split in the delicate perianal skin lining. It primarily occurs due to chronic constipation and the traumatic passage of large, hard, dry stools.

• Symptom Presentation: Patients experience sharp, agonizing, cutting pain during defecation that persists for several hours afterward, leading to a fear of passing stools.
• Bleeding Pattern: Rectal bleeding is typical but distinct, appearing as a bright red streak of blood on the surface of the hard stool or directly on the toilet paper.

Clinical Focus: Treatment centers on immediate internal stool softening to prevent re-injury, combined with accelerated local wound-healing protocols to close the painful fissure track.

3. Fistula-in-Ano (Bhagandara)

An ano-rectal fistula is a chronic, abnormal inflammatory track forming a tunnel that connects an internal opening within the anal canal or rectum to an external opening on the perianal skin. Because this track is lined with unhealthy granulation tissue or tough fibrous tissue, spontaneous healing is highly difficult.

• Clinical Presentation: It initially manifests as a painful perianal pimple or abscess that bursts, resulting in continuous or intermittent bloody or pus-filled discharge in the perineum region. Clinical evaluation and probing reveal a deep track tracking into the anal canal.
